Lingelbach Eighth Grade letter 

Permission Forms

 

  • School Trip Permission Form - all students going on school trips will receive from their teacher a new school district permission form to go on the trip. This form must be completely filled in, signed by a parent or guardian and returned to the teacher before a student can go on any school trip.  (File will open with Adobe Reader.)
  • Permission Form for Use of Student Image (on the School District of Philadelphia Website) - This form is a request for permission to use the picture, voice, video, work and/or full name of your child on the official School District of Philadelphia World Wide Web page on the Internet.

 

Extracurricular Activities

All students participating in after school extra curricular activities must have a permission form signed by their parent or guardian returned to the activity sponsor (e.g., teacher) to attend. Each activity sponsor will give out the form.  These programs are offered at various times throughout the year.  Look for information about clubs which begin in November.  Sponsors will send home information regarding subject matter and grade.

Hello Parents,

To help you get a head start on the school year, below, listed by grade are the supplies your child will need for the up coming year.

 

Grade K : A regular size bookbag (big enough to hold a folder)

                One Instant hand sanitizer with pump

One Elmers School Glue ( 4oz)

                 One tissue box

3 composition books- no spiral notebooks

One container of cleanser / disinfecting wet wipes

  • $2.00 fro Parent Communication Folder 

 

           (teacher will order)

 

  • 5 black/white compostiion books labelded with your child's name and room number (NO spirals)
  • 1 box of 24 crayons
  • 3 folders
  • 1 box of tissues
  • 1 large eraser
  • 1 glue stick
  • A box that will hold all supplies 
  • THREE SHARPENED PENCILS- EVERYDAY 

 

            * These items will stay IN school.

 

  • Crayons, pencils and a pencil sharpener

 

             * These items should be kept at home. 

 

  • Scissors and pencil sharpeners are NOT allowed in school at ANYTIME. 
  • NO magic markers- they are too messy!
