Kindergarten Registration:

    Kindergarten registration is going on now.

Registrations are processed between 8:30am - 3:00pm. 

The following documents are required in order to register your child for Philadelphia public schools:

Proof of student's age: this may include birth certificate, Baptismal or other religious certificate containing the child's birthdate, passport

Current Address:  this may include voter registration card, notorized lease, utility bill sent to new address, driver's license

Up-to-Date Immunizations:  this may include health passport, baby book containing doctor's notes regarding inoculations, physical exam report from doctor

General School Supplies:

1 ream of copy paper

1 bottle of waterless hand sanitizer

1 box of facial tissues

Emergency Contact Form

Please complete this form and return it to your child's teacher. It will provide the school with pertinent information in case of an emergency.

School Trip Permission Slip

This form is to be filled out in completely in order for your child to attend a school sponsored trip.
