Graduation and Promotion Requirements

Graduation and promotion requirements for all School District of Philadelphia students.

 

Permission Forms

  • Physical Forms - The School District of Philadelphia requires your child have an annual physical and update records on file. This form should be taken to your child's health physician and then submitted to the school every school year.
  • School Trip Permission Form - This form must be completely filled in, signed by a parent or guardian and returned to the teacher before a student can go on any school trip.
  • Permission Form for Use of Student Image on the School District of Philadelphia Website - This form is a request for permission to use the image and/or voice of your child on the official School District of Philadelphia World Wide Web page on the Internet.
  • Extra Curricular Activities - All students participating in after school extra curricular activities must have a permission form signed by their parent or guardian returned to the activity sponsor (e.g., teacher) to attend. Each activity sponsor will distribute the form. Please ask the homeroom teacher if you did not receive the form.

Home and School Association

The Cayuga Home & School Association is a cooperative effort of both the staff and parents for the growth, development and education of children.This Association enables parents/guardians to become more involved in their children’s education at school by

  • raising funds
  • providing volunteer help to enrich the school’s program
  • serving as a sounding board for parents’ concerns

 Through its fund raising efforts, the Home & School Association provides

  • programming for Mommy & Me Meetings
  • participation in classroom trips
  • Graduation and Safety Awards

As a service organization, the Home & School Association

  • collects food for needy families at Thanksgiving time
  • provides refreshments at special meetings
