Accelerated School Intake Form

Click on this link to download the Accelerated School Intake Form, which is available for any accelerated school when a student enrolls directly in the site. Please complete this form for all new intakes, and indicate if the student has been assigned or placed on a waiting list. The completed form should be forwarded to the Re-Engagement Center.
