Paperwork

We are pleased to welcome newly hired principals to the School District of Philadelphia!  Read the following information carefully as it will assist in expediting employment. The staff of the Office of Talent is available to provide assistance.

Please complete the following forms and bring them to your processing appointment:

Please obtain and bring the following documents to your processing appointment:

  • Criminal History Check: Apply for your Criminal Record check online at PATCH, using your credit card for payment. Print your results and bring them with you to your meeting with Ms. Sisto.
  • Child Abuse Clearance: Apply for your Child Abuse Clearance through the Pennsylvania Department of Public Welfare. This requires a $10.00 money order. You can find the application form here.
  • FBI Fingerprint Clearance: Please go to COGENT for information on FBI Fingerprint processing. Register through the PA department of Education. Fingerprinting can be done at the District’s Central Office as part of your employment processing or can be obtained from an outside site.
  • Medical Form: To be completed by your physician. You can find the health form here.
  • Transcripts: You must submit official sealed transcripts documenting your degree(s).
  • Professional Certificate: Instructional staff must have their original professional certificate.

If you desire to place a spouse and/or dependents on your benefit coverage you will also be asked to provide the following as applicable:

  • Original marriage certificate
  • Original birth certificates for your spouse and/or dependents
  • Original social security cards for your spouse and/or dependents
  • Original letter of verification from the educational institution for students ages 19 through 26.
