The links below provide methods to access and acquire OGDC forms and applications:

Internal Intent to Apply - This form must be completed prior to submission of any grant proposal that exceeds $20,000.

Outside Entity Intent to Apply - This form must be completed and submitted prior to applying for any grant proposal requiring School District involvement for which the District is not the Lead Applicant.

Revised EITC Outside Entity Intent to Apply -  This form must be completed and submitted prior to applying for approval as an Education Improvement Organization eligible for EITC funding.

Implementation Checklist for Managers - This checklist has been designed to assist grant program managers and coordinators successfully implement their grant awards.

Questions to Consider - This questionnaire has been designed to assist schools and program offices decide whether or not to pursue a grant opportunity.
