Contract Forms

Limited Contract Form - use for contracts under $25,000 (SRC resolution also needed for contracts from $15,000.01 to $25,000).

Agreement for Services - use for contracts over $25,000 after SRC approval of resolution.

Standard Amendment Agreement - use to increase funding, extend the term, or alter the services a vendor provides.

Contractor Information Form -  use to solicit from the vendor the information you need to complete an Agreement for Services or Standard Amendment Agreement.

Criminal Background Check - needed for all vendor employees; Pennsylvania State Police report of all criminal convictions.  If employee has conviction, consult statute (or an OGC attorney) to determine whether the vendor may or should employ the individual.

Child Abuse Clearance Check - needed for all vendor employees; Pennsylvania Department of Public Welfare report of incidents involving actual or possible abuse of children.  If report shows incidents, make a judgment (please feel free to consult an OGC attorney) about whether the vendor may or should employ the individual.

FBI Clearance - needed for all vendor employees; Federal Bureau of Investigation report of criminal activity.  If report shows such activity, make a judgment (please feel free to consult an OGC attorney) about whether the vendor may or should employ the individual.

Affidavit - permitted under Pennsylvania law pending receipt of required background checks for one period only not exceeding 90 days, provided employee has no direct contact with children.  Employee must promptly order and deliver background checks upon receipt; employee certifies that he or she has done nothing that would be reflected on the background checks.

Addendum for Student Funds Contracts Form - Use this form to contract for things like proms, class trips or events, yearbooks, class rings or photography - expenditures, that is, from the school's student funds account(s).
