Contract Forms

Limited Contract Form - use for contracts up to $20,000.00; you must generally send these contracts to the Limited Contracts Committee for approval.  

Standard Terms and Conditions - for use with the Limited Contract Form.

Limited Contract Amendment  – use to modify a Limited Contract, for example to extend the term or add money to the deal.  

Agreement for Services - use for contracts over $20,000.00, after SRC approval of a resolution authorizing the contract.

Standard Amendment Agreement - use to increase funding, extend the term, or alter the services a vendor provides. 

Memorandum of Understanding Form – use to accept programs and services at no cost to the School District.  

Memorandum of Understanding Standard Terms and Conditions – use with the Memorandum of Understanding – imposes requirements for Providers to get background checks, carry insurance and other standard requirements.  

Contractor Information Form -  use to solicit from the vendor the information you need to complete an Agreement for Services or Standard Amendment Agreement.

Criminal Background Check - needed for all vendor employees who have “direct contact” with children; Pennsylvania State Police report of all criminal convictions.  If employee has conviction, consult statute (or an OGC attorney) to determine whether the vendor may or should employ the individual.  Note:  in 2011 the General Assembly significantly broadened the employment bans in this law.  “Direct contact” means routine interaction and the possibility of care, guidance, supervision or control of a child.  

Child Abuse Clearance Check - needed for all vendor employees; Pennsylvania Department of Public Welfare report of incidents involving actual or possible abuse of children.  If report shows incidents, make a judgment (please feel free to consult an OGC attorney) about whether the vendor may or should employ the individual.

FBI Clearance - needed for all vendor employees; Federal Bureau of Investigation report of criminal activity.  If report shows such activity, make a judgment (please feel free to consult an OGC attorney) about whether the vendor may or should employ the individual. 

Affidavit - permitted under Pennsylvania law pending receipt of required background checks for one period only not exceeding 90 days, provided employee has no direct contact with children.  Employee must promptly order and deliver background checks upon receipt; employee certifies that he or she has done nothing that would be reflected on the background checks.

Addendum for Student Funds Contracts Form - Use this form to contract for things like proms, class trips or events, yearbooks, class rings or photography - expenditures, that is, from the school's student funds account(s).
