Step 1. Document Completion.

Have the following documents completed and in your possession:    

• Registration for Per Diem Services Application;
• An original Pennsylvania Teaching Certificate.  if you do not hold a PA certificate follow instructions below:
  • A Bachelors Degree with two (1)  official transcripts that state your degree has been awarded. Transcripts must have a raised seal and must be in separate envelopes;
  • General Application for PA Certificate (Form PDE-338G) . Please note that this form requires your physician's signature needed in section 3, and a five dollar money order;
• Original PA Criminal Background Check (current within 1 year);
• Original PA Child Abuse Clearance (current within 1 year);
• Document(s) establishing employment authorization pursuant to Form I9 requirements.
• School District of Philadelphia Medical Form
• FBI Clearance (Cogent Systems)
