Prekindergarten Head Start Enrollment
Prekindergarten Head Start Enrollment
- When?
- Register your child during the year before you want him or her to attend PreK Head Start
- How?
- Telephone your local center and ask to speak to the Family Services Representative, or
- Attend advertised, scheduled city-wide Registration Days
- Obtain a PreK Head Start Application packet from your local center
- Telephone the Office of Early Childhood Education at 215-400-4270 and ask to have a PreK Head Start Registration packet mailed to you
- What Information is Needed?
- Copy of child's Birth Certificate
- Copy of child's Social Security card (optional)
- Verification of family income (copies)
- Income Letter (other sources) e.g. SSI, Child Support, etc.,or,
- TANF Number (if applicable)
- Completed PreK Head Start PreRegistration Form
- Child's immunization record and Health Assessment (immunization record alone will be accepted for the first 60 days)
- Dental form completed by dentist or date of dental appointment
- Child's IEP or CER (if applicable)
- Important Note: Immigrant families will not be asked for their green cards.
