Bright Futures Enrollment

When?
- Register your child during the year before you want him or her to attend Bright Futures
How?
- Telephone the Office of Early Childhood Education, PreK-3, at 215-400-4270, or
- Attend advertised, scheduled city-wide Registration Days
- Obtain a Bright Futures Application packet from your local center

What information is needed?
- Completed Bright Futures Application
- Child's immunization record and Health Assessment (immunization record alone will be accepted for the first 60 days)
- Child's dental information
- Verification of child's date of birth
- Verification of Philadelphia residency
- Important Note: Immigrant families will not be asked for their green cards.
