  • United Concordia Dental

We are pleased to announce a change in eligibility for Dental coverage for CASA, SPAP and Non-represented employees. Effective February 1, 2012 you can add or keep your children on your dental policy until they turn 26.

Please visit UnitedConcordia.com, choose the "For Members" option and log into "My Dental Benefits" using these directions or call United Concordia Customer Service at 1-800-332-0366 to verify the status of your dependent(s).

This is a qualifying life event change. The enrollment period to add or update the dependents on your coverage will end March 2, 2012. If your dependent is NOT active on your policy, please complete this form and submit to Employee Benefits.

 

Dental Plans Offered

 

CASA, SPAP, and Non-Represented employees have their dental coverage provided by United Concordia.  To locate a participating dentist on the United Concordia site, click "Find a Dentist" and select the "Advantage Plus Network." 

You can access the dental plan's benefit booklet, which describe your benefits, covered services, and excluded or limited services from the United Concordia site.  By logging in at www.UnitedConcordia.com you canview your benefits and eligibility information, claim details, procedure history, maximum and deductible accumulations and print ID cards.
