The federal and state governments require that The School District automatically release your name, gender, address, telephone number, and school name to military recruiters (unless you fill out a student opt out form). You may download this form (in English and in translation) and directions on how to submit it by using the link below. Regarding the translations, you may still use the 2009 translated forms. You may sign and return the form yourself since a parent signature is not required. The signed form must be returned to your adviser by November 30, 2012.
