How to Prevent Student Names and Addresses from Being Released to Military Recruiters

The federal and state governments require that The School District automatically release your name, gender, address, telephone number, and school name to military recruiters (unless you fill out a student opt out form). You may download this form (in English and in translation) and directions on how to submit it by using the link below. Regarding the translations, you may still use the 2009 translated forms. You may sign and return the form yourself since a parent signature is not required. The signed form must be returned to your adviser by November 10, 2011. 


For Parents

Military Opt-Out Form: Parents of 11th and 12th Grade Students  

The federal and state governments require that The School District automatically release your child's name, gender, address, telephone number, and school name to military recruiters (unless you or your child signs an opt out form). You may download this form (in English and in translation) and directions on how to submit it by using the link below. Regarding the translations, you may still use the 2009 translated forms. Note that 11th and 12th grade students may sign and return the form themselves or you can sign it if you prefer. The signed form must be returned to your child's adviser by November 10, 2011.
